Definition: An uncommon inflammatory cyst associated with mandibular molars, typically occurring in individuals under 20 years of age.
Radiographic Features:
- Location:
- Furcation region of mandibular molars
- Most common: Permanent first mandibular molar
- Less common: Second and third molars
- Edge:
- Well-defined
- May be corticated
- Shape: Round to ovoid
- Internal:
- Radiolucent
- Unilocular
- Number: Can be single or multiple
Key Diagnostic Signs:
- Radiolucent lesion in molar furcation area
- Associated tooth tipped lingually (roots towards lingual)
- Lingual cusps appear higher than buccal cusps clinically
Clinical Significance:
- Can cause tooth displacement or delayed eruption
- May lead to expansion of buccal cortical plate
- Associated teeth typically remain vital
- Treatment often involves surgical enucleation and monitoring
Note: Also known as paradental cyst or mandibular infected buccal cyst. CBCT imaging can provide better visualization of tooth tipping and lesion extent. Differential diagnosis from other periapical or periodontal lesions is important for proper management.
